Note: By submitting this form, you and/or your company are agreeing to the following:

• To pay the call out fee of $750+gst

• To pay any additional costs associated with your call out.

• All costs associated with the recovery of unpaid invoices will be passed on to the truck owner.

• If you or your company don't have an account with us, a cash invoice will be created and sent, these invoices will be payable upon receipt.

• If you or your company has an account with us, the account will be billed, and you will pay per your usual payment terms.
